Submitting Documentation

​Here are some tips on the documentation you'll need and ways to submit items.

How to submit the required documentation:

There are several ways you can submit your documentation.
Please be mindful that we must be able to read the documents.

  • You can scan or send a picture from your phone or Office Lens app to
  • Mail documents to
    DMACC at the Evelyn K Davis Center

Required Documentation:

  1. Household Income

We will accept a copy of one of the following:

  • Paycheck stubs-1 month
  • If you receive public assistance (SNAP, SSI, SSDI, HUD), we can accept documentation from them
  • If you (or spouse/significant other) are unemployed, you can provide one of the following:
    • white sheet or wage screen sheet from Iowa Workforce Development (200 Army Post Road/Des Moines)
    • Letter verifying that you are receiving unemployment

What qualifies as a household:

  • If you are married or have a significant other, we need income verification for each of you for the past 6 months.
  • If you are living with family or friends, you will be considered a household of one, plus any dependents you are responsible for.
    • We do NOT need your parents/roommates/etc. income information.

2. Proof of High School Completion

  • We will accept an unofficial graduation transcript or diploma from your high school OR college.
  • If you do not have these records, you can  contact your high school or school district:

3. Proof of State R​esidency

We will accept a copy of one of the following:

  • ​Iowa driver's license
  • A postmarked envelope with your name and address (i.e. utility bill)
  • Voter’s registration card
